Shin Splints in Young Athletes: Causes, Load and Recovery

Shin splints is a symptom, not a diagnosis. In young athletes it almost always means running volume has outrun the leg's current tolerance, and it responds well to a structured rebuild.

What it usually is

The common version is medial tibial stress syndrome: irritation of the bone and surrounding tissue along the inner edge of the shin, spread over several centimetres. It typically aches at the start of a run, eases a little once warm, and is sore afterwards. Pressing along the inner shin reproduces it over a broad area rather than one point.

In young athletes it usually follows a change: preseason, a new sport, a growth spurt, a switch from grass to hard courts or a stretch of training in shoes that have done too many miles.

Signs that need assessment

Pain you can cover with one fingertip, pain that gets worse rather than better as the run continues, pain when hopping on that leg, night pain, or pain that has stopped a young athlete running altogether. These raise the question of a bone stress injury or stress fracture and should be seen by a clinician. So should calf pain with numbness, pins and needles or a hard, tight, swollen compartment during exercise.

The first three weeks

  • Reduce running volume by roughly half rather than stopping. Keep the sessions that do not aggravate it.
  • Shift some conditioning to bike, swimming or rowing to keep fitness while the shin settles.
  • Check footwear age and suitability, and reduce time on the hardest surfaces.
  • Start calf and foot strength work three times a week.
  • Track how the shin feels each morning, not just during runs.

Rebuilding running

Once the shin is quiet on daily activity, rebuild running in small increments: roughly 10 to 15 percent more volume per week, one variable at a time, with at least one non-running day between the harder sessions. Speed work goes in last, once easy volume is comfortable.

Most young athletes with straightforward shin splints are back to a full week in four to eight weeks. Bone stress injuries take longer and need a proper diagnosis first, which is exactly why the pinpoint pain pattern is worth taking seriously early.
